post op case of meningomyelocele

A 2 yr old child was operated for meningomyelocele in vizag sometime ago now the MRI shows tethering of the cord and the doctors have advised the parents to take the baby to AIIMS.I have the scanned copies of the MRI.Kindly help us.

my mother got fits in her brain

hello sir, my mother got hernia surgery before 15days, during the surgery she got massive heart attack and this is the 1st time, simultaneously she got fits in brain, she is still unconsious. she is Diabetic and BP patient she is also suffering with obesity. At present her condition is still unconsious itself sometimes she is opening her eyes and blinking. she is in Sai Bhavani Super speciality hospital, Shapurnagar, Jeedimetla. Doctor said she will recover for sure but it takes much time. Sir what step we can take further.? pls suggest me.

MRI OF BRAIN. REPORT
TECHNIQUES:
TSE-T2 Axial & Sagittals
T2- Flair Axial & coronals
TSE T1 WI Axials

Findings:alteration in morphology&signal pattern of supratentorialcompartment evidence as:
1.Moderante ventriculomegaly involving lateral ventricles with horse shoe. 2.complete absence of septum pellucidumwith falttenngof roof of frontal horns.
3.partial foramtion of separate cerebral hemisphere is evident with thr presence of interhemispheric fissure posterily & narrow deficient anterior IH fissureat thr levelof frontal horns,where continuity of gray & white matter across the midline is seen.3.corpus callosum is thinned& dysplastic.
4. Pituitary fosa is shallowwith thin pituitary stalk(to correlate clinical/hormonal parametersfor endocrine dysfunction.)
5.Azygous ACA presents.
6.Posterier fossa structure includingn 4th ventricle are unremarkable.
7. midline facial anamolies are evident as median clif lip & cleft palate.

IMPRESSION:feature are consistent with semilobar holopresencephaly with moderate lateral ventricular dilatation..

For clinical correlation: adv high resolution CT PNS WITH RECONSTRUCTIONFOR EVALUATING MIDLINE FACIL ANAMOLIES.

Please give me the good suggestion on the report OF MRI. We should go for the operation immeditly and what will be the post complecation after the operation and what will be the cost of operation and medical expenceses. Who will she will lead the life after the operation. It is a cancer or normal toumar. Please help me in this regards.She as given a birth to Baby one month back she can feed the baby.

TECHNIQUE :-

TSE - T 2 Axial, coronal and Sagittals

MR Spectroscopy

Post contrast T 1 - Axial, Coronal & Sagittals

FINDINGS :-
A small circumscribed mass noted at the left frontal cortex with broad base towards the dura,

involving the left, middle frontal gyrus, hyperintense on T 2 with areas of signal suppression

on flair, hypointense on T 1 WI, showing no significant enhancement on post contrast studies .

The mass measures 4 . 8 x 4 . 0 x 5 . 7 cm in the longitudinal, transverse, AP diameters

respectively, there is evidence of gyral thickening at the periphery of the lesion .



No significant mass effect noticed .


However, the adjacent cortical sulcii are effaced .



Rest of cerebral parenchyma is normal in signal intensity pattern with normal

gray - white matter differentiation .



Ventricular system is normal .



Brainstem structures and cerebellum are normal .
